About Transition AIM
Transition AIM is a specialized provider of consultation and educational services dedicated to helping students with disabilities successfully transition from high school to adult life. We recognize that transition planning often happens in disconnected pieces between schools, families, and agencies, leaving students unprepared for the next chapter. Our mission is to bridge this gap by providing coordinated, student-centered transition support that brings together the people, resources, and planning students need to move confidently into adulthood.
Through collaboration with families, schools, and support agencies, we help students explore career pathways, build practical workplace and life skills, and access meaningful opportunities after high school. Every service we provide is designed with outcomes in mind: preparing students for further education, competitive employment, and active participation in their communities. Our approach focuses exclusively on transition services, ensuring students receive expert guidance tailored to their individual strengths, interests, and life goals.
Our Services
Career Exploration & Pathway Planning
We help students discover their strengths, interests, and career aspirations through guided exploration and personalized pathway planning. Our services include career assessments, job shadowing opportunities, and development of individualized transition plans aligned with student goals.
Workplace & Life Skills Development
We provide targeted instruction and coaching to build practical skills students need for success in employment and independent living. This includes workplace readiness training, social-emotional skill development, daily living skills, and community navigation.
Coordinated Transition Planning & Agency Collaboration
We facilitate seamless coordination between schools, families, and support agencies to ensure comprehensive transition planning. Our team helps connect students with post-secondary education opportunities, employment services, and community resources to support their success after graduation.
